Not on [the official BSA site](http://www.scouting.org/forms.aspx). I [found a copy on a Samoset Council](http://samoset.doubleknot.com/resources/forms-documents/59742) doubleknot page.

[I really want this too](https://community.trooptrack.com/t/eagle-palm-applicaion/4008/3) and am willing to put some time into it if I can get some clarification from TroopTrack.
