All I see in my interface is a check button indicating a permission slip is required. Can I do more? Can I type up the form so everyone can print it out? Can they give permission electronically?

Apparently there has been no reply in 3 and a 1/2 years. greaaaaat.
Why are permission slips so difficult to print? Is there a template somewhere that I’m missing?
Today all I’m getting is a “Sorry it’s not working” message. I would be happy with an empty form to fill in.
