I wonder how many others would have a use for a “questionnaire” version of a Checklist. I see a couple use cases.

Voting - An assignment to selected users to respond with their selection among a set of options. i.e. Please vote on options for our Annual Trip: [] Space Camp, [] USS Yorktown, etc.

Feedback - A request for data from each assigned user. i.e. Please enter your shirt size [____]

As with checklists, these would have assignees and due dates. The creator would be able to see the resulting data and maybe even download it as a CSV.

“questionnaire” version of a Checklist

Awesome idea! We would like to be able to gather feedback on aspects of our Annual Summer Camp. Would be great to be able to do that here, instead of Survey Monkey.
