Permission Slip for all events in scouting year

For youth events our church has gone to sending out a permission slip that covers all youth events for the entire year - so you don’t have to fill one out for every event.

We’d like to adopt that plan for scouts - have a slip that parents sign that covers events from X date to X date (August 2016 - May 2017 for example).

We would still want permission slips available for each event - for guests, etc… but would eliminate the hassle of getting individual slips for every scout event during the year.
