Unit-specific Permission Slips for outings

There should be a way for each Troop to upload their own Permission slip and get it to auto-fill like the one TT has. You use the “official BSA” form, we have something much simpler.
Below is an example of ours:

Troop Activity: Overnight Campout at Loud Thunder Cabin
Date/Time of Departure: Friday, January 20th, 2017; 5:00 pm
Date/Time of Return: Sunday, January 22nd, 2017 '11:00 am (ish)

My son, ______________________________, has permission to participate in the activity named above. I understand that he will be expected to represent his troop and follow the Scout Law to the best of his abilities.

Cost: $12 ____ Check/Cash Attached ____ Please take from my son’s account
____ I am willing to participate in this activity as an adult chaperone. I can take _____ passengers, including my son.

NOTE: All Scouts should travel in their Class A uniform and remember to bring your scout handbooks. This campout is in conjunction with Klondike Derby. Scouts must dress as appropriate for the weather, expecting to spend Saturday outside doing strenuous activities. Extra gloves and warm socks are recommended.

Our troop uses a custom permission slip with some additional fields. We would welcome this change as well.

OR…allow us to upload our own pdf as a permission slip.
